Midway in our fourth year five-to-seven-year initiative, Healthier Jupiter is a community-driven initiative funded by Palm Health Foundation. Headquartered at Jupiter Medical Center, Healthier Jupiter works in partnership with the Medical Center to address healthy behaviors, social determinants of health, and quality of life issues across the lifespan. Healthier Jupiter brings the community together by listening to the needs of all, building support, and providing resources to empower people to make healthier choices. Healthier Jupiter is part of Palm Health Foundation’s Healthier Together Initiative. Healthier Together is a long-term, place based funding approach designed to solve the community’s most complex healthcare issues, neighborhood by neighborhood.
As with any long term initiative, an evaluation framework is important to ongoing monitoring, impact assessment, and learning. To that end Healthier Jupiter provides an Impact Report to the community and our generous funder Palm Health Foundation every six months. This report shares: who we are, why diabetes prevention is a focus, where we have gone so far, key lessons learned, what we have done in the past six months, and next steps in the initiative. Click here to download the report: Healthier Jupiter Impact Report (April 2018 – September 2018).
We appreciate the continued support.
