Local leaders and residents in the Boynton Beach community have identified Family Caregiving as the critical health issue in their area and are collaborating with Palm Healthcare Foundation, utilizing the committed funds, to create lasting, impactful change.

Jupiter, Delray, Boynton Beach, North Palm Beach/Riviera, Lake Worth and The Glades were selected to receive $200,000 a year for the next five years in an effort to improve the health and well-being of local residents. Each community selects from Palm Healthcare Foundation’s three priority areas for Healthier Together - diabetes prevention and management, behavioral health and family caregiving.

Goals of the 5-year place-based initiative:

  • To help change long-term health outcomes of the residents in Palm Beach County
  • To increase capacity among individuals, organizations, and systems to impact lasting and sustainable
    change related to health and well-being
  • To establish a learning framework to evaluate the overall impact of the Healthier Together initiative as
    well as the impact at the community level
