Healthier Jupiter has partnered with Living Hungry Coalition to launch a nutrition intervention Pilot Program in Jupiter. Living Hungry Coalition designed this comprehensive nutrition wellness intervention program to improve the health of those in our community living with food insecurity and diagnosed with diabetes or pre-diabetes. The program allows partners: Jupiter Medical Center, MyClinic and C.L. Brumback Jupiter Clinic, to refer patients in this pilot demonstrating health outcomes can be improved by changing what individuals eat. Food as Medicine interventions have confirmed improvements in outcomes in patients with pre-diabetes, diabetes, heart disease and other chronic diseases.
The Living Hungry Coalition Pop Up Eat Well Center program is designed to provide patients access to fresh food, nutrition knowledge, and positive food related habits. The pilot program will run from June-September and support 60 patients in Jupiter. Patients attend the pantry every other week and will be supplied healthy fresh groceries chosen by each patient, following a custom menu of options. Cooking classes and nutritionist on-site for 1:1 coaching and counseling. Patients will be screened pre and post intervention to provide program measurement.
Healthier Jupiter is thrilled to be part of this innovative pilot program with a collaborative group of partners: Diabetes Coalition of PBC, C.L. Brumback Jupiter Clinic, Jupiter Medical Center, MyClinic, Living Hungry Coalition, The Poverello Center, and South Florida Hunger Coalition. Funding provided by the Healthcare District Palm Beach County.
Volunteers Needed every other Saturday, 8:30 am – 1pm, for the Living Hungry Coalition Pop Up Eat Well Center. Dates: July 28th, Aug 25th, Sep 8th, Sep 22nd , Sept 29th.
